Your issues could be related into each other. Your Assist might stop responding because of your issue [2], it crashes before it sends you the error about it, or if it wont stop responding you might end up to issue [2].
That other guy had same error than you with his bluetooth headset, he didn't get same error when he used another sound source. I suggested you to try using different sound source than your usb soundcard, maybe this is what you already tried. Some one else suggested you to check your video card driver

I have list of connected accounts with Lion Adrian from like year ago, there's actually over 30 accounts from where only half are used in Assist (at that time, dunno what is the situation now, haven't seen him around). You were playing at that time as J_Rambo and I thought you played with him, but your account was not on the list of his linked accounts back then.

But you're speaking in general, that's what people in ts can do. Using ts doesn't automatically mean ghosting, like you stated you're not even talking about that matter. In same team if it's reported that enemy is in west wing behind the desk ( or if you know Adi is playing and he wasn't at ambush spwan, then he's there at west wing
) you can kind of surround him from both ways. It's teamwork which makes this game more interesting than playing it alone, you can discuss about various kind of stuff, you make new friends etc. This is why people should use more ts imo
