nvidia 9600 GT?
There could be lot of reasons.
The most probably are: 1) overclocking problem, 2) overheat
I have very similar problems, when OC my CPU not correctly.

Did you try to overclock your GPU or CPU?

No I didn't.
-
What about temperatures of CPU?
If you have dual ram memory, try to uninstall one of them.

CPU's temperature is about 40 C.
I think if the problem was in memory there would be other issues too..?

Dowlonad 3dMark 06 and try to do it couple times. If it will run correctly, the problem isn't hardware.

Not at first but on second time it did. That text on the right corner means: "graphic card stopped responding and recovered (came back) "

I know that feeling, I had the same problem.
My problem was incorrect CPU overlock.
In your case, like I said, it may be couple of reasons.
You eliminated graphic driver, because you reinstalled or updated it.
How long do you have this problem?

I didn't play for three years.. I started to play about three weeks ago and the problem started four days ago..

This is hardware problem in my opinion.
But before "hardcore" actions, try to reinstall or to update chipset and motherboard drivers. All, every one driver for every one device.

Well, I formated my Windows today and re-downloaded all drivers.

Problems with voltage?
Damaged RAM/GPU/CPU?
Outdated BIOS?
-
it this only happend at aa25 or do you generaly have this problem?
if only at aa25, tray to change the grafic settings in there - over aa25assist panel - settings...
in not, than do you have this version of grafic software installed? if not, do so.
http://www.geforce.com/drivers
be sure that you install the right driver there (32/64bit, notebook or desktop, language..) and install now the newest one (you can also try beta-drivers, its ok)
still not good, bring card back to vendor
